Position Summary
Responsible for assessing, planning, coordinating, and evaluating the care of patients.
Scheduled Hours40 hours per week.
PrimaryDuties & Responsibilities
- Conduct a comprehensive sensitive assessment with the client and family/support network as appropriate, in order to develop a plan of care to meet identified needs.
- Arrange and coordinate service referrals for the client/family as needed; provide ongoing monitoring of the activities implemented to carry out the plan of care. Refer clients/families to public benefits programs as eligible.
- Provide regular contact with enrolled clients and document all contacts and service coordination activities in client records as required by program guidelines.
- Educate clients/families about the basic medical and psychosocial aspects of disease in order to optimize adherence to care.
- Provide proactive follow‑up, tracking symptoms and treatment responses.
- Adhere to all applicable state laws and the department’s rules and regulations regarding programs.
- Stay abreast of new resources, programs and developments. Participate in continuous training and education, including attendance at staff meetings and community meetings as assigned.

Master’s degree or combination of education and/or experience may substitute for minimum education.
Certifications / Professional LicensesBasic Life Support – American Heart Association;
Basic Life Support – American Red Cross.

Salary Range$52,600.00 – $78,900.00 per year.
