• Illinois state social work license or eligibility to obtain it within 90 days

Under the direction of a physician, the Social Worker assists patients and their families in their residences or inpatient/outpatient settings to adapt and plan an interdisciplinary approach to the patient‑family system.

• Provides direct clinical services to patients and families.
• Plans and implements social work services on assigned clinical services or units in collaboration with the healthcare team.
• Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.

• Education: Master's Degree in Social Work.
