Onsite - Community Health Worker

A community health worker is a front-line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Duties are performed in a manner consistent with Mercy Mission, Values, and Mercy Service Standards.

• Perform duties and responsibilities in alignment with Mercy mission, values, and service standards.
• Serve as a liaison/link between health/social services and the community to facilitate access to services.
• Support culturally competent service delivery and community engagement.

• Education:
  
  High School Diploma or equivalent
• Experience:
  
  1 year health care or social services experience
• Strong relationship and collaboration skills
• Excellent communication skills
• Ability to work independently
• Compassion for poor and underserved members of the community
• Basic Microsoft Office skills

• Community Health Worker Certification
• Additional language skills as applicable to community
