OverviewA community health worker is a front-line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Duties are performed in a manner consistent with Mercy Mission, Values, and Mercy Service Standards.
Responsibilities- Perform duties and responsibilities in alignment with Mercy mission, values, and service standards.
- Serve as a liaison/link between health/social services and the community to facilitate access to services.
- Support culturally competent service delivery and community engagement.
- Education:
High School Diploma or equivalent - Experience:
1 year health care or social services experience - Strong relationship and collaboration skills
- Excellent communication skills
- Ability to work independently
- Compassion for poor and underserved members of the community
- Basic Microsoft Office skills
- Community Health Worker Certification
- Additional language skills as applicable to community
