Overview

BJC Home Care is looking for a Registered Nurse (RN) for home health in the Saint Louis, MO area. The role focuses on direct patient care, work-life balance, and schedule flexibility, with opportunities to build meaningful relationships with patients.

• Assess patient preferences and barriers to involvement in care, including values, emotional, spiritual, cultural, and population-specific needs.
• Develop, implement, and document individual plans of care with defined goals in collaboration with the interprofessional team, patient, family or caregiver, and in accordance with established nursing standards. Proactively communicate the plan of care across the continuum of care.
• Promote respect, equity and empathy in care delivery, including support for emotional, spiritual, and cultural preferences; participate in collaborative problem-solving and advocacy for patient- and family-centered continuity of care. Integrate interprofessional input and critical thinking into care delivery.
• Evaluate changes in patient condition, inform and collaborate with family and/or caregivers, and communicate changes with the interprofessional team; update the plan of care in the EMR and ensure evidence-based practice and quality outcomes.
