Park Nicollet is looking to hire a Registered Nurse – Transition Liaison to join our Homecare team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary

The Transition Nurse Liaison will be responsible for coordinating the care of patients in Health Partners’ enterprise Hospital post-acute care network whose medical condition indicates that they will benefit from Home Care/Infusion Services. This position will assume primary coordination responsibility for Home Care/Infusion referrals to Methodist Home Care (MHC) and/or Park Nicollet Home Infusion (PNHI). The Transitional Nurse Liaison will work closely with pharmacy, nursing, social services, physicians, rehabilitation services, patients, and their families.

Required Qualifications
  • Education, Experience or Equivalent Combination:
    • Minimum of two years, hospital-based acute care nursing experience.
    • Two years’ experience in a home care setting.
  • Licensure/ Registration/ Certification:
    • Current Minnesota R.N.
    • License BLS provider status in accordance with American Heart Association (AHA) guidelines
    • Current MN driver’s license
  • Knowledge, Skills, and Abilities:
    • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
    • Requires basic computer/keyboarding skills.
Preferred Qualifications
  • Education, Experience or Equivalent Combination:
    • BSN preferred.
    • Previous hospital-based case management experience strongly desired
    • Nursing experience in specialty areas including, but not limited to oncology, neurology, or orthopedics.
  • Knowledge, Skills, and Abilities:
    • Knowledge of how hospital departments interact and how patients flow through a hospital setting.
    • Knowledge of external post-acute and sub-acute health care providers/systems/options in the Twin Cities and greater Minnesota.
    • Knowledge of word processing, spreadsheet, and presentation software desired.
