Registered Nurse – Home Care/Hospice
Park Nicollet is looking to hire a Registered Nurse to join our Home Care/Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day. courthouse
Position SummaryThe Home Care/Hospice registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community based upon community standards of practice. The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self‑directed and/or assisted care in their community setting.
The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.
Monday – Friday (with 1 regular day off per week), every 6th‑7th weekend. Monthly on‑call, bi‑annual weekend on‑call. 2 holidays per year.
LocationCentral Team – Minneapolis/ St. Paul
Required Qualifications- Education, Experience or Equivalent Combination:
Work experience as an RN for at least two years. - Licensure/Registration/Certification:
Current MN Registered Nursing License. - Knowledge, Skills, and Abilities:
Demonstrates knowledge of age‑related growth and development principles necessary to provide decorators. - Proficiency with personal computer, telephone, fax machine, photocopier, printer and electronic medical record.
- Experience in home care and/or hospice desirable.
- Must be able to function independently with minimal supervision. < kuva>
- Demonstrates initiative, judgement, critical thinking, and the ability to problem solve.
High level of sensitivity, assessment skills, technical skills and communications skills.
