The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
Responsibilities- Work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Work closely with patients on a 1-1 level but receive strong support from a patient care team.
- Act independently on day-to-day tasks, including phone, email and written communication, meetings with a variety of healthcare professionals, and understanding of hospice rules and regulations.
- Manage time effectively for visits, documentation, meetings, prioritize and triage multiple concerns.
- Learn electronic medical record systems.
- Drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area.
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
