• This position assists in all facets of the referral process (preadmission, admission, follow-up and discharge). This position assesses, plans, implements, evaluates, and supervises individual patient care following established standards of care. Accepts assignments and travels to homes to deliver care. This position builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
  
  This position provides on-call coverage during non-business hours of operations.
• This position works under the supervision of the Associate Director and/or Lead RN and is employed by Mosaic Health System.

• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illnesses. Documents all care timely.
• Regularly re-evaluates patient nursing needs. Reports changes in patient condition to physician. Obtain/receives physician’s orders as required.
• Develops and initiates the plan of care and makes necessary revisions as patients’ status and needs change. The plan of care is based upon patient assessment and physicians’ orders. Uses health assessment data and OASIS/HIS data to determine nursing diagnosis.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, compassionate, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.

• Nursing Diploma or Degree from an accredited school of nursing is required, BSN preferred.
• Current certification or licenses as required by state law within the practicing state. BLS certification required within 90 days from start date. Valid in-state driver’s license is required upon hire.
• Minimum of one-year acute care or public health experience;
  Home Care experience preferred.
