Summary

• This position evaluates, plans, and implements treatment programs for the rehabilitation of patients, as directed by a physician, using approved treatment procedures. Provides patient and family education and participates in family conferences. Responsible for all necessary documentation.
• This position works under the supervision of the Lead RN and is employed by Mosaic Health System.

Duties

• Provides physical therapy services to patients according to a written physician’s plan of care.
• Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
• May train patients in the use of prosthetic devices.
• Identifies patient and family/caregiver needs for other home health services and refers as necessary.
• Other duties as assigned.

Qualifications

• Graduate of an accredited physical therapy program. All required education is a minimum requirement. Higher levels of education are acceptable.
• Licensed Physical Therapist in the State of Missouri, BCLS - Basic Life Support, and Valid Driver's License in the State.
• 1 Year of appropriate experience as a physical therapist required. 1 Year of community/home health experience preferred.
