Role Overview

Private Duty Nurse (PDN)‑LPN is a member of the client care team that provides services directly, or acts as a placement agency, to deliver skilled nursing services to clients in their personal residences. The PDN LPN works under the supervision of the Clinical Manager.

Reporting To: Clinical Manager
Wage/Hour Classification: Non‑exempt
Scheduling: Flexible hours, case‑by‑case basis

Job Responsibilities
  • Evaluates and treats clients with appropriate level of skill according to client’s needs under the supervision of an RN, Clinical Manager.
  • Assist with providing continuity of care and emotional needs of client and family.
  • Medication Administration.
  • Resourceful in meeting client’s psychosocial needs.
  • Verbal demonstration of client rights and responsibilities.
  • Make decisions based on sound nursing judgements.
  • Recognizes and copes with emergencies appropriately.
  • Ensures proper management of client services.
  • Demonstrates competent use of equipment.
  • Appropriately communicates information relative to client care in accordance with agency policies.
  • Observes the client’s general physical, emotional, and mental status, and reports all changes in the client’s conditions, family situation, incident, and occurrences during shift immediately to agency RN/CM or Customer Care Coordinator.
  • Ensures all forms of communication with clients, families, and health plan providers are documented timely to promote continuity of care across all interdepartmental functions.
  • Build relationships with clients to encourage compliance with care plans and client safety.
  • Maintains accurate clinical and shift work documentations to update system in accordance with guidelines set forth by the agency in a comprehensive and timely manner.
  • Manages client/contract satisfaction with the highest level of customer service standards, communication, and documentation.
  • Adheres to infection control policies and exposure control plan as mandated.
  • Monitors and maintains clinical report, including Infection and Occurrence/Complaint Reports.
  • Adheres to and promotes safety and injury prevention measures in client and workplace settings.
  • Maintains and improves skills through attendance with staff development programs and in‑service education requirements.
  • Participates in community outreach activities that promote the goals and objectives of the agency.
  • Maintains confidentiality in all professional areas and considers the needs of clients and their families, other employees, physicians, visitors, and all members of the community in an informative, courteous, and compassionate manner.
  • Ensures all HIPAA regulations are adhered to.
  • Follows agency policies and procedures.
  • All other duties as assigned.

ADA Requirements
  • Routinely required to sit, stand, and walk on variable surfaces, including occasional climbing of ramps or stairs or the use of foot controls.
  • Routinely required to use hands for various tasks, including hand controls, gripping, and handling of objects.
  • At times, will be required to reach beyond mid‑line of the body, reach overhead or at chest level.
  • Specific vision abilities are required, including close, distance and peripheral vision, depth perception and ability to adjust focus.
  • All employees will be required to use universal safety precautions when performing their assigned tasks.
  • Employees must have the ability to lift, push, pull and carry a minimum of 25 pounds.
