The RN Case Manager plays a vital role in ensuring the health, safety, and well-being of home care patients through comprehensive assessments, individualized care planning, ongoing monitoring, and staff supervision. This role supports each patient in achieving their highest level of independence while maintaining compliance with state and federal regulations.
Required Skills & Abilities- Strong knowledge of adult clinical care and home care regulations
- Proficiency with Windows-based systems and healthcare software
- Excellent planning, time management, and ability to work independently
- Strong written and verbal communication skills (English proficiency required)
- Professional demeanor with integrity, flexibility, and confidentiality
- Commitment to service excellence and teamwork
- Ability to manage multiple tasks, prioritize effectively, and problem-solve
- Strong attention to detail and ability to recall information accurately
- Comfortable working independently and in isolated settings when needed

- Handles patient and staff concerns with professionalism
- Current Registered Nurse (RN) license in the State of Minnesota
- Two years of experience in home health, med-surg, or acute care preferred
