SummarySt. Cloud VA Health Care System is seeking a Hospice and Palliative Care Advanced Practice Nurse to care for Veterans in St. Cloud, MN. The incumbent practices in Rehabilitation and Extended Care (REC) Integrated Clinical Community (ICC) Service and is responsible for the assessment, planning, coordinated provision, and evaluation of individual treatment plans for Veterans receiving care from the Hospice Palliative Care program.
Duties- Licensed independent practitioner with advanced education and national certification who provides evidence‑based care for a population of patients.
- Provides a full range of services emphasizing quality, timeliness, and the advancement of the VA mission.
- Provides advanced clinical practice, consultation, and management.
- Responsible for planning, integrating, implementing, modifying, and evaluating the effectiveness of the Veteran’s care.
- Leader of the integrated care team established by St. Cloud VA Health Care System.
- Serves as a liaison to internal and external providers and manages the care of the Veteran and seamless transition to and from the community.
- Knowledge of VHA guidelines, utilization review criteria, evidence‑based standards of care, compliance guidelines, and local policy related to delegation of authority as established by the facility Chief of Staff.
- Participates in unit/program level quality improvement processes, initiatives, and customer service programs.
- Follows the standardized care coordination model, incorporates case management principles if applicable, and has knowledge of referral and authorization processes and secure medical documentation exchange.
- Identifies Veterans requiring disease management and obtains resources internally or externally as applicable.
- Focuses on providing care unique to veteran needs, monitoring, tracking, and intervening in the episode of care across the continuum.
- Demonstrates effective communication skills to collaborate and co‑manage the Veteran’s care with the multidisciplinary team in a cost‑effective manner.
- Provides care and counseling of persons in the promotion and maintenance of health and prevention of illness and injury based on the nursing process that includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through services such as case finding, health teaching, and health counseling.
- Provides supportive or restorative care to maintain life and well‑being.
- Executes medical regimens, including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider.
- Primary commitment to the patient – whether an individual, family, group, or community.
