We are seeking a Physical Therapist or Occupational Therapist who is Lymphedema Certified to join our home health team and provide specialized care to patients managing lymphedema in the home setting. This role is ideal for a clinician who is passionate about improving quality of life through skilled, compassionate, and evidence-based care.
Therapist can manage a mixed caseload of home health patients, including individuals with lymphedema as well as patients with orthopedic, neurologic, post-surgical, chronic condition, and functional mobility needs.
Key Responsibilities- Perform comprehensive evaluations for patients with lymphedema
- Develop and implement individualized plans of care
- Provide Complete Decongestive Therapy (CDT) including:
- Manual lymphatic drainage (MLD)
- Compression bandaging and garment education
- Skin care education
- Therapeutic exercise
- Educate patients and caregivers on self-management techniques
- Accurately document care in the EMR
- Communicate with physicians and interdisciplinary care teams
- Ensure compliance with home health regulations and agency standards
- Assess, treat, and monitor skin integrity issues and wounds commonly associated with lymphedema, including:
- Weeping edema
- Venous stasis ulcers
- Pressure injuries
- Moisture-associated skin damage (MASD)
- Cellulitis prevention and early identification
- Perform basic to moderate wound care as ordered, including cleansing, dressing selection, and compression-compatible wound management as it pertains to lymphedema
- Licensed Physical Therapist (PT) or Occupational Therapist (OT) in the state of practice
- Current Lymphedema Certification (required)
- Home health experience preferred (but not required)
- Strong clinical assessment and patient education skills
- Reliable transportation and ability to travel within the service area
- Compassionate, patient-centered approach to care
Employment Type
