- Instructs, assists, and supports recipients in areas such as interpersonal communication skills, community resource utilization and integration skills, crisis assistance, relapse prevention skills, health care directives, budgeting and shopping skills, healthy lifestyle skills and practices, cooking and nutrition skills, transportation skills, medication education and monitoring, mental illness symptom management skills, household management skills, employment‑related skills, and transition to community living services.
- Ensures immediate and appropriate interventions are provided in response to changes in mental status, symptoms or behavior that may put the recipient at risk, and changes must be made in the Individual Treatment Plan.
- Establishes therapeutic relationships with assigned recipients and meets with each client as planned; maintains a good understanding of therapeutic boundaries.
- Reflects a respectful and appreciative philosophy of cultural and other differences encountered when interacting with co‑workers and in serving recipients.
- Drives to client homes or various community locations where services are provided, and occasionally transports recipients at supervisor's direction.
- Completes initial and ongoing training requirements defined for the position to meet required standards.
- Prepares DLA‑2 assessments, Mental Health Information System (MHIS) reporting and develops individual treatment plans for assigned recipients; completes individual crisis plans annually and reviews and revises each document as required.
- Develops and maintains recipient files according to Minnesota Statute 256B.0623; documents all services provided accurately, completely and within required deadlines.
- Participates in discharge planning for recipients being discharged from an Intensive Residential Treatment Services facility or inpatient hospital when Transition Community Living (TCL) is authorized.
- Coordinates care and participates in collaborative treatment planning meetings, whether within CMMHC’s programs or with another entity identified in the Certified Community Behavioral Health Clinic (CCBHC) statutory language related to care coordination.
- Participates actively in staff meetings and supervision.
- Has at least 4,000 hours of supervised experience in the delivery of services to adults or children with mental illness, substance use disorder, emotional disturbance, traumatic brain injury or developmental disabilities and completes training on mental illness, recovery from mental illness, mental health de‑escalation techniques, co‑occurring mental illness and substance abuse, and psychotropic medication side effects.
- OR an individual qualified as a mental health practitioner through relevant coursework (at least 30 semester hours or 45 quarter hours in behavioral sciences or related fields) and has at least 2,000 hours of experience providing services to individuals with mental illness, substance use disorder, traumatic brain injury or developmental disability; completes the additional training described in section 245I.05, subdivision 3, paragraph (c) before providing direct contact services.
- OR holds a master’s degree or other graduate degree in behavioral sciences or related fields.
- OR holds a bachelor’s degree in behavioral sciences or related fields and completes a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields.
