Licensed Practical Nurse

The Licensed Practical Nurse position is located in Specialty Care, Nursing Specialty, Nursing Ambulatory Care, Geriatrics & Extended Care, Nursing Geriatrics & Extended Care, Nursing Mental Health & Mental Health at the VA St. Louis Health Care System. The LPN is to perform assigned duties in a manner that promotes quality health care, outstanding customer service, and the maintenance of an environment committed to serving veterans.

Major duties and responsibilities of the position: The Licensed Practical Nurse performs under the guidance of the Nurse Manager or designee. Assigned tasks require basic technical competence and skill. The LPN is accountable for all patient care and must function within the LPN scope of practice as it relates to Primary Care policy and procedures. The LPN is responsible for observing, recording, and reporting changes in the patient's condition as well as assisting in the development and revision of patient‑driven holistic life care plan.

The LPN will initiate appropriate actions based on the treatment plan and their observation of patient condition under the supervision of RN Care Manager. LPN demonstrates active collaboration and communication with the Core Team (PCP, RN Care Manager and MSA) and expanded team. The LPN assists with providing patient and family health education with focus on self‑management, prevention and wellness.
