Licensed Practical Nurse (LPN) location:
Cranberry Park at ShorePointe, 26101 Jefferson, St. Clair Shores, MI, 48081.
Position Overview:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Cranberry Park ideal candidate will have strong medication administration and documentation skills, experience in geriatric nursing, and a patient-centered approach to care. This role requires the ability to recognize and respond to changes in resident conditions, particularly in those with chronic diseases and dementia-related illnesses.
- Administer medications accurately and document all nursing activities in compliance with regulatory standards.
- Provide nursing care focused on geriatric patients, including chronic disease management and dementia/Alzheimer’s care best practices.
- Monitor residents’ health status and promptly recognize and respond to any changes or emergencies.
- Communicate effectively with residents, families, and multidisciplinary team members to ensure coordinated care.
- Supervise and support care staff to maintain high standards of resident care.
- Maintain accurate and detailed records using electronic health records (EHR) systems; basic computer skills required.
- Demonstrate strong organizational and time-management skills to manage multiple priorities efficiently.
- Apply crisis management and problem-solving skills to handle challenging situations calmly and effectively.
- Ensure compliance with all regulatory requirements and facility policies.
- Current LPN license in the state of Michigan.
- Strong medication administration and documentation skills.
- Experience with geriatric nursing and chronic disease management.
- Knowledge of dementia and Alzheimer’s care best practices.
- Ability to recognize and respond to changes in resident condition promptly.
- Excellent communication and interpersonal skills.
- Compassionate, patient-centered approach to care.
- Strong organizational and time-management skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Basic computer skills; experience with electronic health records (EHR) preferred.
