Rehab Nurse | LPN

As a Rehab Nurse | LPN, you are responsible for performing restorative nursing procedures that maximize the resident’s existing abilities, emphasize independence instead of dependence, and minimize the negative effects of disability in accordance with current federal, state, and local standards governing the community.

• Provide restorative nursing care to residents in accordance with clinical standards and regulations.

• Support residents to maximize independence and functional abilities.

• Collaborate with care teams to develop and implement individualized care plans.

• Maintain accurate documentation of care provided and resident progress.

• A fire and passion for working with seniors
• A flexible, fun, and energetic personality
• Must have active, and in good standing, Licensed Practical Nurse (LPN) license in the state of practice
• Prior experience as an LPN in a LTC/SNF/AL/MC setting preferred
• Previous experience or completion of appropriate training program in rehabilitation and restorative therapy practices
• Professional image in both appearance and behavior
• Excellent written and oral communication skills
