Geriatric Care Manager; RN or LCSW

Overview

The Independent Contractor Care Manager at Aging Gracefully develops and implements a comprehensive and individualized plan for clients. The scope of care management can be wide and varied as the Care Manager oversees care management and advocates for client needs.

Key Responsibilities

• Conduct comprehensive assessments addressing medical, functional, psychosocial, cognitive, and environmental needs.
• Develop and implement individualized care plans based on client goals and available resources.
• Coordinate and monitor services such as home care, medical appointments, transportation, housing, and community programs.
• Advocate for clients’ rights and preferences across care settings.
• Maintain regular communication with clients, family members, and care providers to ensure consistent support and satisfaction.
• Provide crisis intervention and assist families in navigating complex care decisions.
• Document all activities and maintain accurate, timely client records.
• Participate in team meetings and case conferences to ensure continuity and quality of care.
• Stay current on aging-related issues, benefits programs, and community resources.

Qualifications

Education:

Master’s degree in Social Work or Nursing.

Experience:

• Minimum of 2–3 years of experience working with older adults, individuals with disabilities, or in healthcare/social service settings.
• Previous experience in care coordination, case management, or aging services preferred.

Licensure/Certification (preferred):

• Licensed Social Worker (LSW/LCSW), Registered Nurse (RN), or Certified Care Manager (CMC or GCM) designation preferred.
