Overview
The Independent Contractor Care Manager at Aging Gracefully develops and implements a comprehensive and individualized plan for clients. The scope of care management can be wide and varied as the Care Manager oversees care management and advocates for client needs.
Key Responsibilities
- Conduct comprehensive assessments addressing medical, functional, psychosocial, cognitive, and environmental needs.
- Develop and implement individualized care plans based on client goals and available resources.
- Coordinate and monitor services such as home care, medical appointments, transportation, housing, and community programs.
- Advocate for clients’ rights and preferences across care settings.
- Maintain regular communication with clients, family members, and care providers to ensure consistent support and satisfaction.
- Provide crisis intervention and assist families in navigating complex care decisions.
- Document all activities and maintain accurate, timely client records.
- Participate in team meetings and case conferences to ensure continuity and quality of care.
- Stay current on aging-related issues, benefits programs, and community resources.
Qualifications
Education:
Master’s degree in Social Work or Nursing.
Experience:
- Minimum of 2–3 years of experience working with older adults, individuals with disabilities, or in healthcare/social service settings.
- Previous experience in care coordination, case management, or aging services preferred.
Licensure/Certification (preferred):
- Licensed Social Worker (LSW/LCSW), Registered Nurse (RN), or Certified Care Manager (CMC or GCM) designation preferred.
Skills and Competencies
- Strong assessment, advocacy, and problem-solving skills.
- Compassionate and person-centered approach to care.
- Excellent communication and interpersonal abilities.
- Strong organizational skills with attention to detail and follow-through.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Proficient with electronic documentation and Google Workspace.
