What You Will Do
  • Conduct psychosocial assessments by interviewing the patient and their significant others to obtain history and identify preliminary issues for treatment focus.
  • Contact appropriate previous and current tertiary caregivers to obtain relevant collateral clinical data that may be needed for the patient’s coordination of care, treatment planning and discharge planning.
  • Develop, implement and monitor progress of the patient’s individualized treatment plan.
  • Conduct daily group and individual assessments to identify issues for treatment focus by interviewing the patient one-on-one and transcribing relevant information.
  • Provide individual, group and family process‑oriented counseling sessions. Conduct educational, after‑care support and other didactic groups for patients and family members using various professional treatment modalities.
  • Participate in regular clinical team meetings/staffing and provide input on the psychosocial, disease management and case management needs of the patient.
  • Demonstrate an understanding, and adhere to all procedures and legislation, which apply to patients cared for in a Mental Health and Substance Abuse treatment facility.
  • Demonstrate a comprehensive knowledge of community resources (12‑step support meetings, recovery homes and residential treatment facilities) and medical allied staff expertise to assure an appropriate discharge liaison is implemented.
  • Report to community/state agencies any patient issues that are required by hospital policy and state or federal law. Serve as the facilities representative in legal proceedings resulting from said reports.
  • Maintain accurate and timely medical records including psycho‑social assessments, family session and daily progress notes to include school personnel and other community collateral involvement. Provide descriptions of therapeutic interventions and patient response for discharge planning process.
  • Schedule, attend and document court hearings as required, such as involuntary admission, guardianship, etc.
  • May provide backup services for incoming assessment and referral calls.
  • Participate in activities that enhance professional growth and development. May serve as a spokesperson before professional or community groups.
  • Demonstrate conduct in keeping with LOH ethical standards.
  • Comply with facility safety, infection control and security program.
  • Float to alternate units or Linden Oaks locations based on census and staffing needs.
What You Will Need

Required Education, Skills and/or

Experience:

  • Master’s Degree in Social work or a Master’s Degree in a recognized mental health field or a Bachelor’s Degree and completion of an approved substance abuse education program for assignment in the substance abuse area.
  • Knowledge of developmental psychology, disease pathology and clinical needs of assigned population.
  • Ability to maintain information which is highly confidential.
  • Ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation.
  • Ability to demonstrate self‑control in volatile or hostile situations.
  • Ability to set therapeutic limits and maintain therapeutic distance with clients; maintain therapeutic boundaries with patients during treatment and following discharge.

Preferred Education, Skills And/or

Experience:

  • Minimum of two years of experience in a behavioral health setting.
Required License And/or Certification
  • Current State of Illinois licensure with a minimum of three letters which may include LPC (Licensed Professional Counselor), LCPC (Licensed Clinical Professional Counselor), LSW (Licensed Social Worker), Associate Licensed Marriage and Family Therapist (ALMFT), LMFT (Licensed Marriage and Family Therapist), LCSW (Licensed Clinical Social Worker), or LCP (Licensed Clinical Psychologist) unless primary patient population is chemical dependency when a CADC is required.
