Overview
The North Florida/South Georgia VA Healthcare System's Social Work Service is seeking an experienced and dynamic Senior Social Worker (Caregiver Support). Ideal applicants are those who want to contribute to the achievement of quality care standards, as well as the growth and development of a VA academic hospital program. You must be an excellent communicator and team player who is adaptable to the needs of a growing medical center.
Responsibilities- Provide clinical evidence based case management services and interventions.
- Management of application processing.
- Caregiver, Veterans and staff education on caregiver issues.
- Community outreach and resource development.
- Continuous quality improvement activities and evaluation/consultation across the North Florida / South Georgia Veterans Health System (NF/SGVHS).
- Work within the Social Work Service located in Saint Augustine, FL.
