Family Medicine Physician

The Family Medicine Department in Jacksonville, Florida is seeking a Board Certified / Board Eligible Family Medicine physician to join the practice at the St. Augustine Clinic. This role is heavily involved in innovative primary care models. Workflows emphasize and prioritize preventive care, disease screening, and asynchronous care provided by each member of the care team. There is a large focus on virtual and RN care to meet acute and chronic patient needs.

The team is led by a Family Medicine Physician who provides patient care in conjunction with their paired Nurse Practitioner or Physician Assistant.

• Experience in advanced care models in Family Medicine in residency or practice is preferred but not required.
• Night call approximately 1:15 with a similar rotation for weekend clinic coverage

Qualifications
:
Candidates must be Board Certified / Board Eligible in Family Medicine and have or able to obtain a Florida Medical License. Three years’ experience is strongly preferred; will consider new resident graduates.
