Northwestern Medical Center is currently recruiting a Physical Therapist in Rehab Service (Georgia Urgent Care office). The position is full-time (72 hours bi-weekly), generally M-F 7a-5:30p. Hourly Range: $32.12 USD to $46.38 USD

Job Summary
  • The Physical Therapist provides physical therapy services for inpatients and outpatients across a spectrum of ages and within the continuum of care. Evaluates clients, plans and implements treatment programs which restores, reinforces, and/or enhances function.

  • Education:

    Graduate of an accredited school of Physical Therapy with current licensure in the State of Vermont.

    Director of Rehab Services

    Patients, families, visitors, providers and their staff, business partners and hospital employees
Scope
  • Machinery or Equipment Used:
    Ambulatory aids, therapeutic laser, ultrasound, electrical stimulation, exercise equipment and other patient care equipment. Computer and photocopier.
  • Physical Demands:
    Frequent, prolonged standing/walking. Lifts, positions, pushes, and/or transfers patients, occasional reaching, stooping, bending, kneeling and crouching. Manual dexterity and mobility and moderate physical effort.
  • Working Conditions:

    May be exposed to infections and contagious diseases, occasional exposure under a wide variety of circumstances. Subject to varying and unpredictable situations. Subject to many interruptions. Occasional pressure due to multiple calls and inquiries.
  • Required Protective Equipment: PPE as situation warrants
Essential Functions
  • Provides exceptional patient care to our community within NMC & APTA standards of care evidenced by:
    Flexibility in schedule to meet the needs of our patients;
    Adapts information for diverse populations and environment;
    Advocates for needs of patients;
    Provides care in ethical and legal manner;
    Refers concerns/issues to appropriate resources;
    Protects confidentiality of patients;
    Communicates with patients, team members, and management in an accurate and timely manner.
  • Provides efficient patient care within NMC & APTA standards of care. Meets productivity standards of >= 67% (solid= average of 67% for running 12 month period)
  • Promotes NMC Rehab through community outreach event and/or process improvement with cross discipline teamwork.
  • Meets Documentation Standards:
    Completes timely clinical documentation/order entry;
    Complies with payer guidelines/limitations;
    Re-administration of Outcome measures demonstrating patients are making meaningful progress meeting dept expectation of 90% re-admin rate for discharged charts with >= 8 visits.
  • Applies Critical Thinking Skills And Considers Alternatives:
    Prioritizes Tasks/Organizes multitask workload;
    Demonstrates problem solving and decision making skills;
    Offers assistance to others in critical thinking and seeks assistance as needed
  • Committed to on-going learning and practice of evidence based medicine:
    Successful chart audits demonstrating need for skilled services and appropriate treatment progression;
    Up-to-date research on best practices evident in peer chart audits
  • Incorporates teaching in delivery service:
    Provides appropriate information to patient;
    Assesses for barriers to learning;
    Uses colleagues as a learning resource;
    Supports the preceptorship program
  • Manages self and resources effectively:
    Acknowledges limits of experience and capability;
    Identifies learning needs and seeks resources;
    Utilizes equipment and supplies appropriately;
    Maintains clean, orderly environment of care;
    Seeks out and accepts feedback;
    Requests assistance appropriately
