- 2+ Years of Experience - [REQUIRED]
- TELE CARDIAC Experience - [REQUIRED]
- Recent traveler experience - [HIGHLY PREFERRED]
- Candidates' Permanent Address (Facility will consider local candidates - 50-mile radius restriction, for ONLY 1-13week contract)
- ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION (Facility will NOT approve scheduling requests at/after time of offer / Please obtain this information prior to submittal / Requests after offer can be cause for rescinded offer)

- Work History
- Skills Checklist, current to 1 year, pertinent to the speciality (Telemetry) - [REQUIRED] [DOCUMENT]
- 2 Supervisory Reference Forms Completed within the last two years - [REQUIRED] [DOCUMENT]
- Active American Heart Association BLS - [REQUIRED] [DOCUMENT]
