Essential

Job Duties
  • Accept referrals from the care team regarding patients’ behavioral health needs, respond in a timely and collaborative manner, and provide and document referrals as needed to appropriate providers/resources both within and outside GLBHC.
  • Provide comprehensive assessment, diagnosis, consultation, and interventions for behavioral health, substance use, or social determinants of health needs. Recommend referral to a different specialty provider, other member of the care team, or consultation as indicated. Provide counseling/behavioral health services to patients individually, couples, family, and/or group basis.
  • Assist the care team in developing care management processes such as guidelines, disease management techniques, case management, and patient education to improve self‑management of chronic conditions (mental and physical).
  • Consult and collaborate with the patient and care team regarding recommended behavioral health interventions, communicating recommendations to both patient and team.
  • Assist in the detection of “at risk” patients and develop plans to prevent further psychological or physical deterioration, using available screening tools, stressor assessments, and diagnostic codes to determine risk level or referral need.
  • Meet standards for quality and utilization and follow GLBHC policies, procedures, and expectations.
  • Maintain pertinent, accurate, and timely documentation.
  • Assist in monitoring behavioral health needs at the assigned site and make recommendations to the immediate supervisor regarding gaps or opportunities for improvement.
  • Attend and actively participate in huddles, staff meetings, case conferences, and meetings that support GLBHC behavioral health care models to provide comprehensive care for patients.
  • Display a positive and professional attitude, interact well with colleagues and leadership, and approach the job with enthusiasm and passion for serving GLBHC patients.
  • Provide regular supervision to school‑based therapists.
  • Other duties as assigned.
Job Specifications
  • Education:

    Master’s degree in social work or counseling from an accredited graduate school. Basic Life Support (BLS) certification required.
  • Licensure:
    Licensed as a Licensed or Limited Licensed Master’s Social Worker or Licensed Professional Counselor in the State of Michigan. BLS certification required. SUD:
    Michigan Certification Board for Addiction Professionals (MCBAP) certification preferred.
  • Experience:

    Preferred minimum of three (3) years of experience in a behavioral health setting. If Limited Licensed Master’s Social Worker, must obtain license in a timely manner and be actively pursuing. Must be in good standing with licensure expectations.
  • Skills:

    Skilled in written and oral communication, basic computer skills, and ability to work in an electronic medical record.
  • Interpersonal

    Skills:

    Prioritize activities short and long term, handle multiple tasks efficiently, resolve interpersonal conflicts, and develop cooperative relationships with clinical management, executive structure, board of directors, business associates, and community groups.
  • Physical Effort:
    Sit, stand, walk, and lift up to 25 lbs. occasionally.
  • Hours of Work:
    Full time or Part time; flexible and varied.
  • Travel:
    Travel may be required within the service area. Use of personal vehicle is possible. Reimbursement per corporate policy and IRS guidelines.
