Overview
Turning Point Community Programs is seeking Outreach Workers for our Community Outreach Recovery and Empowerment (Core-Rosin) program in Sacramento, CA
. Turning Point Community Programs (TPCP) provides integrated, cost-effective mental health services, employment and housing for adults, children and their families that promote recovery, independence and self-sufficiency. - Dedicates 30 hours per week to street and community outreach at homeless shelters, service agencies, the Loaves & Fishes complex, river areas, under bridges and overpasses and various other places where homeless individuals spend time.
- Reconnects with members who have lost contact with staff for 30 or more days.
- Provides follow-up and continued outreach to members who have voluntarily discharged from the program and who remain at risk.
- Maintains a continuous outreach caseload of 15 non-enrolled individuals.
- Maintains a continuous enrolled caseload of between 3-5 members as needed.
- Makes no less than 30 contacts per week (defined as a face-to-face meeting with an individual who is a focus of outreach services).
- In the absence of the Team Leader, provides leadership and direction for clients as assigned.
- Assists assigned clients to identify and progress toward achievement of their chosen goals.
- Advocates for and assists clients and their families in accessing and receiving mental health treatment, medical care, housing, food and clothing as indicated.
- Develops individual service plans for each client.
- Coaches clients in developing age-appropriate social skills, money management, personal hygiene, and job readiness.
- Interacts productively within the team in order to provide holistic case management services to an identified group of clients.
- Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.
- Attends to the safety, health, and well being of clients.
- Provides after hours, “on-call” coverage to program clients.
- Provides support to other staff members as needed.
- Enforces the policies and procedures of Turning Point Community Programs.
- Attends all team and agency staff meetings unless approval for non-attendance is secured by the Team Leader and the Program Director.
- Drives on company business and may transport members or other individuals who meet the criteria for the target population.
- Other duties may also be assigned.
- Meets the standard for consistent attendance (not more than a 5% absence rate per month) by reporting to every scheduled shift on time and ready to work and by the judicious use of paid sick leave and avoiding any situations resulting in the need to dock pay for time not worked.
Monday - Friday, 8:00 am - 4:30 pm
Compensation$21.00 - $22.29 per hour
