Case Manager

Meals on Wheels Sacramento County is a nonprofit organization providing vital services to the Sacramento County community. Based in Sacramento, California, the organization is focused on supporting the well-being and independence of older adults. This is a full-time, on-site position located in Sacramento, CA, for a Case Manager at Meals on Wheels. The Case Manager will assist clients in accessing services and resources, conduct assessments to determine needs, develop care plans, and ensure clients receive appropriate support. Additional responsibilities include maintaining accurate records, collaborating with a multidisciplinary team, and following up with clients to evaluate the effectiveness of services provided.

• Case management and client service skills, including client assessment, care planning, and follow-ups.
• Strong communication, interpersonal, and relationship-building abilities to collaborate with clients, families, and team members effectively.
• Organizational and record-keeping skills for managing case files and documentation accurately.
• Knowledge of social services and community resources relevant to senior care is highly beneficial.
• Proficiency in basic computer skills and Microsoft Office Suite.
• Associate’s degree in social work, human services, or a related field preferred.
• Ability to work on-site full-time in Sacramento, CA, and to communicate inclusively with diverse populations.
