Family Partner

The Family Partner ensures families are equal partners in the planning, development and implementation of mental health services at the Sacramento Children’s Home. The Family Partner supports that agency policies and systems are family friendly, culturally competent and stigma-free at all levels of the agency. The Family Partner works on integrating advocacy services into the Wraparound program. The Family Partner provides peer support to eligible caregivers and guidance in linking to and navigating various systems.

(85%) Direct Advocacy Services

• Work directly with assigned caregivers to provide one on one support and peer counseling.
• Participate in the development and implementation of family-friendly and consumer-driven services within new and current programs.
• Participate in treatment planning, case conferences, etc. as appropriate.
• Provide support, coaching and assistance to caregivers with education rights during special education processes as needed.
• Provide client and family with support towards stabilization of housing for the purpose of preventing or addressing homelessness.
• Collaborate with team, clients/caregivers to develop housing plans.
• Develop relationships with landlords.
• Partner with clients/caregivers to address housing barriers and provide resources as needed.
• Provide intensive case management to support self-sustaining housing.
• Provide individual advocacy, education, and support to meet the needs of families, to include identifying systemic barriers to participating in services.
• Provide the parent/caretakers’ perspective and feedback to the clinical staff in order to increase staff awareness of the needs of families and possible barriers to meeting the client’s goals.
• Provide support and advocacy to families navigating various systems (i.e., school, housing, CPS, Alta Regional Center, etc.), as appropriate.
• Provide information to parents/caregivers to help engage them in forming a positive relationship with treatment team members. Support parents/caregivers in self advocacy and in making informed decisions regarding the care of their children.

• Prioritize participation in client and family meetings as needed.
• Document all visits with families/clients and services provided.
• Design, implement and facilitate Caregiver Support group for Caregivers being served in SCH Mental Health programs.
• Utilize existing strengths and strengths to build to advocate for accessibility and inclusion of the child in community activities and resources.
• Assist in the ongoing development and evaluation of the service program

(15%) Other Duties

• Attend meetings and work groups as assigned by supervisor to provide a voice for family needs.
• Attend trainings/workshops/committees to expand knowledge base, network with other community programs, and provide a voice for families around agency policy.
• Carry out mandated reporter duties as required by law.
• Establish and maintain collaborative and effective working relationships with internal staff throughout all levels of the agency and with external resources
• Serve as a role model and advocate for children and families involved in the Wraparound process.
• Adhere to ethical and professional standards in all aspects of service delivery.
• Foster a supportive and empowering environment for children and families to participate in their own journey actively.
• Perform other duties as assigned.

Monday – Friday 9:00am – 5:30pm

$22.46 - $25.27 per hour to start

A High school diploma or GED is required. A BA degree in Social Work, Psychology, or a related field is preferred. Previous experience working in a mental health or social service setting is preferred.

Must have experience as a parent or caregiver of a child with serious emotional disturbances that received service within the public systems including mental health, child welfare or juvenile justice.
