Infection Preventionist / MDS Nurse (LVN)

💰 $43-$45/hour

Midtown Oaks Post Acute in Sacramento is looking for a dedicated, detail‑oriented LVN to join our clinical leadership team as an Infection Preventionist / MDS Nurse

This dual‑role position plays a vital part in ensuring regulatory compliance, infection control excellence, and accurate reimbursement through high‑quality MDS assessments.

You’ll collaborate closely with nursing leadership, interdisciplinary teams, and frontline staff to maintain a safe, compliant, and resident‑centered environment.

• Oversee and manage the facility’s Infection Prevention & Control Program
• Track, trend, and report infections in accordance with CDPH and CDC guidelines
• Conduct infection control surveillance and audits
• Provide staff education and training on infection prevention practices
• Ensure compliance with state and federal regulations
• Serve as liaison during audits and surveys

• Complete and coordinate accurate and timely MDS assessments
• Ensure proper documentation to support reimbursement and quality measures
• Collaborate with IDT to develop and update care plans
• Monitor case mix and quality indicators
• Maintain compliance with Medicare/Medicaid regulations

• Current California LVN license (required)
• MDS experience preferred
• Infection Prevention experience preferred (IP certification a plus)
• Strong knowledge of skilled nursing regulations
• Detail‑oriented with strong organizational skills
• Ability to work independently and collaboratively
