Position Summary

The Care Team Nurse (NCT) reports to the Director of Health Services and provides general nursing care and primary care team leadership, working collaboratively with physicians and multidisciplinary team members. The Care Team Nurse will promote and restore patients’ health by providing, delegating, evaluating, and coordinating nursing care for all patients. This individual may oversee and participate in clinical care and case management of patients.

Essential Functions
  • Assesses, plans, directs, and evaluates total nursing care as determined by the patient’s age-related physical, psychosocial, and cultural needs in accordance with established standards, policies, and procedures of the health center.
  • Promotes patient’s independence by establishing patient care goals.
  • Assists provider with examination and procedures as requested.
  • Provides case management for complex, high‑needs patients by serving as a patient advocate, patient resource, and contact person for patient treatment coordination.
  • May provide health‑related counseling on the prevention of communicable diseases, coordinate health programs with outside organizations and agencies, and introduce general health‑related information and instruction to individuals, families, and community groups.
  • Assesses and evaluates patient’s response to treatment and medication.
  • Keeps accurate patient records; maintains confidentiality at all times.
  • Properly prepares and administers immunizations, vaccines, and therapeutic injections.
  • Assists in research and investigation of communicable diseases when requested.
  • Coordinates referrals to IHSS.
  • Serves as a resource person to other staff in the area of one’s own clinical expertise.
  • Reports communicable disease to the county.
  • Complies with all state and federal laws and regulations, as they pertain to position including HIPAA, sexual harassment, scope of practice, OSHA, etc.
  • Other duties as assigned.
Skills And Abilities
  • Excellent computer skills, preferably with Windows, including Microsoft Office Suite.
  • Excellent telephone and communication skills.
  • Must possess excellent organizational, writing, and verbal skills.
  • Ability to work independently, set priorities, and work well under pressure.
  • Ability to maintain a high degree of confidentiality.
  • Ability to demonstrate superior professionalism when dealing with patients, subordinates, colleagues, community members, and vendors.
