IP/Case Management (Healthcare)

Responsibilities- Administrative Functions:
Direct the day to day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long term care facility. Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility. Periodically review the department's policies, procedure manuals, job descriptions, etc. Make recommendations for revisions. Meet with your assigned nursing staff and support personnel in planning the shifts services, programs, and activities.
Ensure the Nursing Service Procedures Manual is current and reflects the day to day nursing procedures performed in this facility. Ensure compliance with the Nursing Service Procedures Manual and make written and oral reports/recommendations concerning shift activities as required. Cooperate with other resident services when coordinating nursing services to maintain the resident s total regimen of care. Manage adherence to job descriptions and participate in the facility s quality assurance program for the nursing service department.
Participate in facility surveys (inspections) as requested and review the resident s discharge plan, updating it as required. Interpret department policies and procedures to personnel, residents, visitors, and government agencies as required. Admit, transfer, and discharge residents as required. Complete accident/incident reports and forward to the appropriate office. - Administrative & Documentation Duties:
Maintain the Daily Census Report and submit to the Business Office as required. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. Ensure confidentiality and report suspected violations of user IDs, passwords, and resident information to the Administrator. Encourage physician review of treatment plans and orders and transcribe physician orders to resident charts and nursing records.
Chart nursing notes accurately reflecting care and resident response; file required documentation upon admission, transfer, and discharge; forward information to Food Services as needed. Forward transfer forms and report discrepancies to the Nurse Supervisor. - Drug Administration & Safety:
Prepare and administer medications as ordered by the physician, verify resident identity, manage floor stock, order medications and supplies as needed, and maintain narcotic records. Notify the Nurse Supervisor of drug/narcotic discrepancies and ensure proper disposal. Review medication cards for completeness and accuracy, acknowledging automatic stop orders before last dosage. - Personnel & Leadership:
Participate in staff evaluations, determine staffing needs, and make recommendations on personnel actions. Ensure staffing coverage, address absenteeism, develop work assignments, and provide leadership to nursing personnel. Conduct daily rounds to monitor performance and address problem areas with the Nurse Supervisor. Ensure all nursing assistants are trained in approved programs and supervised by licensed staff. Foster a cooperative working relationship with inter-departmental teams to meet resident needs.
Maintain a warm, positive, and safe unit environment; supervise discipline fairly and in compliance with policy, and manage communications during shift changes. - Nursing Care & Resident Services:
Inform staff of admissions, room assignments, and expected arrival times. Prepare rooms for new admissions, accompany residents as needed, orient residents and families, make rounds with physicians, arrange diagnostic and therapeutic services, and implement nursing objectives and standards. Ensure direct nursing care is provided by licensed nurses, CNAs, and/or CNA trainees, and that care plans are followed by staff. Inspect treatment areas for cleanliness and safety, administer procedures, and use equipment safely.
Maintain care plans, monitor resident conditions, and coordinate with physicians and family as changes occur. Notify physicians of incidents, injuries, and condition changes; support restorative and rehabilitative programs; and manage end-of-life and post-mortem procedures per policy. - Safety, Sanitation & Infection Control:
Monitor safety regulations, ensure personal protective equipment is available, and maintain clean, sanitary work and resident care areas. Enforce hand hygiene, participate in fire safety and disaster drills, and support infection control programs.
