POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS – CA RN license; BLS; 3 years hospital experience and needs acute level care coordination experience;
Experience with discharge planning and utilization review;
Experience with and proficient at Midas, Inter Call, Cerner, Curaspan, and Microsoft Office programs;
Prior CA Dignity/Common spirit acute care experience.

PREFERRED QUALIFICATIONS – Minimal Time Off Requests

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8hr Days, EOW

SYSTEMS – Cerner
