Overview

Health Plus Staffing is seeking a Registered Nurse Case Manager to provide care and support to terminally ill patients. This will be a full-time position primarily serving Merced and South Stanislaus Counties.

• Provide compassionate hospice care to terminally ill patients, focusing on symptom management and emotional support.
• Coordinate and deliver patient care as a nurse case manager in the Merced and South Stanislaus Counties region.
• Apply nursing care processes and adhere to medication guidelines and side-effects management.
• Maintain high ethical standards and safety precautions while delivering care and support to patients and families.

• Minimum one year nursing experience; hospice or home health experience preferred.
• Knowledge of nursing care processes and medication guidelines/side-effects.
• Strong commitment to safety, ethics, and patient confidentiality.
• Proficient in reading and writing English.
• Excellent communication skills and a compassionate nature.
• Cool-tempered, patient, with good emotional stability and stamina.
• California Registered Nurse license required.
• CPR certification current.
• Must have reliable transportation and auto insurance.
• Hospice experience preferred.
