The Registered Professional Nurse – Home Therapies RN CAP 1 is an entry‑level designation within the Clinical Advancement Program (CAP). This position is accountable for the provision and coordination of clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. The RN assesses and manages patients’ responses to home dialysis training and therapy, follows predetermined protocols, and communicates patient‑related issues to the physician as needed.
As part of the End‑Stage Kidney Disease (ESKD) health‑care team, the RN participates in decision‑making, teaching, leadership functions, and quality‑improvement activities that enhance patient care outcomes and facility operations.
- Perform all essential functions under supervision, collaborating with Educators, Preceptors, or other Registered Nurses.
- Collect and analyze dialysis data for assigned patients, documenting and adjusting treatment plans as indicated, and notifying the Supervisor or physician when necessary.
- Assess and address patient/family learning needs, providing initial and ongoing education.
- Direct and provide, with collaborating staff, all aspects of safe and effective delivery of dialysis therapy to assigned patients.
- Administer medications as prescribed and document justification and effectiveness.
- Initiate or assist with emergency response measures.
- Serve as a resource for the health‑care team, participating in staff training and orientation of new staff as assigned.
- Ensure correct laboratory collection, processing, and shipping procedures, rescheduling missed or insufficient collections.
- Identify expected outcomes, update the nursing assessment and plan of care through collaboration with the Interdisciplinary Team.
- Ensure patient awareness of transplant and treatment modality options.
- Assist in identifying, evaluating, selecting, and educating Home Dialysis candidates and Home Partners.
- Assess barriers within the Home Dialysis candidate’s environment and partner/family readiness for at‑home therapy.
- Train Home Dialysis patients and/or Home Partners on safe operations of all home dialysis equipment and supplies.
- Complete CAP requirements to advance.
- Perform all other duties as assigned by the Supervisor.
- Provide direct patient care that involves heavy lifting, moving equipment, and patient ambulation; assistance may be required.
- Require frequent, prolonged standing and ability to bend over.
- May need to move machinery and equipment of up to 200 lbs., and lift liquids up to 30 lbs. at a height of 5 feet.
- Work in a health‑care facility with air temperature control and moderate noise levels.
- May be exposed to infectious materials.
- Rotate coverage with other licensed home therapy staff as assigned to ensure reliable coverage.
- Participate in on‑call, night, weekend, holiday rotations as defined by program needs.
- May travel to training sites, other facilities, or patient homes.
- Essential functions may be performed in other locations, including patient homes, with similar physical demands.
- Daily tasks include desk work, computer use, and interaction with patients, staff, and physicians.
- Oversee LPNs/LVNs, RNs, Patient Care Technicians, and Home Therapy Care Team Assistants as a Team Leader or designated Nurse in Charge.
- Must complete all FKC education and training requirements for new employees.
- Require a minimum of 12 months experience as an RN.
- Complete 3 months experience as an RN in home peritoneal dialysis and/or hemodialysis.
- Graduate of an accredited School of Nursing.
- Current state licensure.
- Current CPR and BLS certification.
- Meet practice requirements in the state of employment.
- Entry‑level RN with less than 1 year of nephrology nursing experience in the last two years.
- Minimum of 1‑year experience as a Registered Nurse (preferred).
- Home dialysis therapy experience (preferred).
Hourly Rate: $34‑$53
