Scope

of Work

Capio Group is looking for an experienced Project Manager to be a part of a team to provide Organizational Change Management (OCM) services, consultation, and support for a client Case Management project. The OCM services will help facilitate the successful implementation of the new case management system to support the business operations. The Project will require engagement with internal and external stakeholders, including client staff, other entities, and external providers depending on the needs.

The change management services will include a roadmap for managing change:

  • Baseline Assessment
  • Messaging and Communication
  • Implementation
  • Measuring Success

The Project Manager will be responsible for tasks including but not limited to:

  • Change Management Implementation and Delivery:
    Work collaboratively with the client Project Team to implement the components of the change initiative.
  • Change Management Evaluation and Closure
  • Documentation:
    All OCM documentation, templates, forms, etc. that are a result of this agreement, and associated deliverables must be flexible and reusable by the client for future projects and initiatives. Documentation must include, but not limited to, policies and standards. Additionally agree and adhere to administrative processes supporting action items, issue tracking, risk management, version control, and tracking document changes.
  • Change Management Planning and Assessment:
    Document project goals, assess current state, conduct a gap analysis, develop a plan to bridge the as-is to the to-be state, and develop measures to evaluate progress and effectiveness.
  • OCM Project

    Schedule:

    Develop and deliver an overall project schedule for all OCM activities, tasks, and deliverables.
  • Change Initiative Gap Analysis:
    Develop a Change Gap Analysis document with baseline assessment, readiness, desired state, and recommended actions.
  • Change Initiative Assessment:
    Develop methodology and measures to monitor progress and effectiveness.
  • Change Management Plan:
    Develop a Change Management Action/Communication Plan with actions, milestones, timelines, and success criteria.
  • Change Management Tasks:
    • Establishing branding and core messaging for the change initiative
    • Development and implementation of a strategy to roll out the change to affected stakeholders
    • Development and implementation of strategies to address stakeholder questions and concerns
    • Identification and selection of tools and methods for ongoing communication to client managers and staff and affected stakeholders for project status and progress
  • Change Management Kick-Off Meetings:
    Facilitate a kickoff meeting with the client Project Team and affected stakeholders including purpose, roles, and feedback methods
  • Change Management Communication:
    Develop and execute a plan to provide regular updates to client managers and staff and affected stakeholders on status and progress
  • Initiative Check-In Meetings:
    Convene regular check-ins with the client Project Manager and other identified managers and staff at least monthly
  • Stakeholder Status Updates and Feedback Meetings:
    Convene regular meetings with stakeholders affected by the change initiative at least quarterly
  • Change Management Initiative Success:
    Use agreed metrics to measure success and lead corrective actions
  • Change Management Evaluation Meeting:
    Convene and facilitate meetings to discuss accomplishments and lessons learned, roles and expectations of affected stakeholders, and recommendations for continued change management; provide a Change Management Closure Report
