Role ScopeUnder the supervision of the PACE Executive Director of Senior Care Services, responsible for the overall operations of the PACE center. Works with key management members to achieve services according to licensing and regulatory standards applicable to PACE.
Core Responsibilities & Expectations for the Role- Create a center experience that Participants want to spend time in, and team members love. Establish a team that cares and creates joy, an environment where all participants and team members belong, and lead a service-oriented operation.
- Continue to raise the bar. Constructively seek and share feedback, and quickly implement changes to improve how the center and the PACE program meet goals and mission.
- Establish and honor Habitat values within the team.
- Accountable for the delivery of high-quality nutrition, kitchen, reception, engagement, and personal care services (in-center and in-home) to ensure participants are uniquely cared for on an individual basis.
- In coordination with other stakeholders, manage nutrition, engagement, personal care, housekeeping, and front desk reception operations, including hiring, training, monitoring, and mentoring staff and ensuring clear and effective workflows.
- Coordinate participant needs across PACE Center, home environment, and outside services seamlessly.
- Collaborate with the Executive Director to plan and facilitate effective interdisciplinary team (IDT) and other participant-care related meetings, ensuring they are organized and engaging, and that follow-up items are documented and tracked.
- Coordinate and assist with physical plant, supplies and equipment management, including monitoring ongoing maintenance and vendor oversight, and ensuring a safe, clean, and welcoming physical environment for participants, staff, and visitors.
- With other leaders, drive quality improvement reviews and follow-through, ensure compliance with policies/procedures/regulations, support successful completion and resolution of internal and state audits; with the Executive Director, develop repeatable processes and measurements for success for the organization.
- Experience in working with Senior Care Services and elderly population.
- Experience working collaboratively with nursing, social work, psychology, recreation, occupational therapy, physical therapy, speech therapy, dietetics, gerontology.
- Minimum of three years’ experience in a supervisor or management position in healthcare or related field.
- A minimum of one year experience working with the frail or elderly.
- Proof of medical clearance for communicable diseases, including a TB test.
- Proof of all immunizations are up to date.
- Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law. (if applicable)
- A state issued driver’s license, personal transportation, and auto insurance as required by law
We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $110,000 - $125,000. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
