Overview

Job Summary - The Licensed Clinical Social Worker (LCSW) provides comprehensive, patient-centered social work services tailored to each patient s medical condition, cognitive and emotional functioning, communication abilities, physical capabilities, support system, living environment, and social determinants of health. Services include, but are not limited to:

• Biopsychosocial assessments
• Crisis intervention, conflict resolution, and mediation
• Psychiatric diagnostic evaluations informed by the DSM
• Suicide risk and safety planning
• Supportive and grief counseling
• Mandated reporting of abuse or neglect
• Assistance for victims of violence or human trafficking
• Symptom and harm reduction
• Patient identification and locating next of kin
• Education and support for patients and families related to adaptation to illness, treatment, or life changes
• Facilitating access to community-based resources for patients and families experiencing emotional, social, environmental, or financial stress

LCSWs are available on-site 24 hours per day as needed to meet program requirements.

This position requires flexible scheduling, including variable hours, occasional overtime, weekends, and holidays, to meet the operational needs of the Clinical Social Services department. The LCSW must adhere to department policies and procedures and adapt to changes in workflow, standards, regulations, technology, healthcare coverage, and social work practice guidelines.

The LCSW may participate in special projects, data collection, and quality improvement initiatives. Responsibilities include peer review and individual supervision. Supervisors and managers maintain overall responsibility for assigned work, schedules, performance evaluations, corrective actions, promotions, and dismissals.

Minimum Qualifications

• California Licensed Clinical Social Worker (LCSW) with a valid, unrestricted license from the California Board of Behavioral Science
• Master s Degree in Social Work from an accredited School of Social Work
• Experience with in an acute medical hospital, mental health setting, and/or health care clinic
• Experience working in healthcare or with clients with chronic health conditions
• Proficient in basic computer skills
• Competent in reading, writing, speaking, and comprehending the English language
• Skillful in the appropriate use of medical terminology
• Capable of maintaining composure in high-pressure situations
• Committed to delivering excellence in patient care
• Exceptional written and verbal communication skills for effective collaboration and clear conveyance of complex clinical information
• Accountability and professionalism in all interactions
• Ability to organize multiple concurrent activities, set goals, plan, and meet deadlines while maintaining quality
