Licensed Clinical Social Worker

UCDH Home Care Services Licensed Clinical Social Worker services are provided to individuals and caregivers in their place of residence for the purpose of promoting, maintaining or restoring health, or minimizing the effects of illness and disability. The Licensed Clinical Social Worker (LCSW) provides comprehensive clinical social services that are appropriate to the needs of the individual patient, family, and caregivers, and functions as an integral member of the interdisciplinary team.

Social Worker 2

UCDH Home Care Services Social Worker 2 services are provided to individuals and caregivers in their place of residence or the hospital for the purpose of promoting, maintaining, or restoring health, or minimizing the effects of illness and disability. The Social Worker 2 (SW2) provides comprehensive clinical social services that are appropriate to the needs of the individual patient, family, and caregivers, and functions as an integral member of the interdisciplinary team.

Licensed Clinical Social Worker

• California Licensed Clinical Social Worker (LCSW)
• American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) or UC Davis Health CPR
• Valid California Board of Behavioral Sciences Licensed Clinical Social Worker license without restrictions
• Valid California driver’s license and driving record acceptable per local policies; enrollment in California Employer Pull Notice Program may be required
• Master’s Degree (MSW) in social work from an accredited school
• Experience in an acute medical hospital, mental health setting, or health care clinic
• Basic computer skills
• English language proficiency; strong written and verbal communication
• Ability to use medical terminology appropriately
• Ability to remain calm under pressure
• Commitment to excellence in patient care; accountability and professionalism
• Ability to organize concurrent activities, set realistic goals, and meet deadlines with quality
• Ability to manage conflict and advocate effectively
• Knowledge of legal implications in clinical practice and documentation
• Knowledge of development, behavior, culture, psychotherapy, case management, and medicine relationship
• Knowledge of child abuse, elder abuse, domestic violence reporting requirements, and related laws
• Ability to provide consultation, interpret findings, and advise on treatment and disposition
• Ability to use clinical supervision for professional growth
• Ability to work independently with minimal supervision; strong problem-solving skills
• Experience with psychosocial assessments for pediatric/adult/geriatric populations
• Knowledge of suicide risk assessment and safety planning
• Ability to assess adjustment to illness and provide brief counseling for individuals and caregivers
• Experience with death/dying/bereavement issues

Social Worker 2

Knowledge and understanding of individual, child, and family development, human behavior and communication, the influence of culture and values in social work practice, psychotherapy, case management, and the relationship between social work practice and medicine.

• Ability to provide consultation, interpret clinical findings and diagnosis, and make treatment recommendations
