- Bachelor’s degree in a related field AND
- At least 4 years approved experience*
- Master’s degree in Social Work, Counseling, Psychology with CA-BBS registration as LMFT/AMFT or LCSW/ACSW or
- Master’s degree in related field and at least 2 years or approved experience*
- Writes clear, accurate and timely progress notes consistent with each contact/visit
- Works collaboratively with all agency programs and staff to provide support as needed
- Attends weekly staff meetings, supervision and case management staff meetings as scheduled
- Accountable for independent time management
- Develops and maintains positive relationships with agency and community programs
- Utilizes and maintains Microsoft Outlook calendar with all work-related details in order to manage time effectively and share calendar information with co-workers
- Utilizes resources to utilize behavior interventions and strategies
- Forms positive, constructive relationships with children and their families
- Communicates effectively with all members of the Child and Family Team
- Maintains positive relationships with consumers and customers
- Identifies strengths, skills, and positive qualities in children and families
- Identifies temporary and permanent sustainable resources and services within family and community environments
- Assists families with transitions from resources provided or funded by Wraparound program to resources and services provided or funded by the family and community
- Participates in the Internal Utilization Review process and other peer-review mechanisms, providing feedback to colleagues, accepting feedback from colleagues, and promptly responding to recommendations
- Serves as role model for the wraparound process, demonstrating particular commitment to family safety, unconditional care, family voice and choice, team-driven decision-making, strength-based planning and interventions, flexible services and supports, and community partnership and collaboration
- Provides leadership in the overall assessment, development, implementation and evaluation of individualized wraparound plans
- Coordinates the development of Child and Family Teams (CFTs), maximizing the involvement of natural team members and system partners
- Attends and guides the weekly CFT meetings through the plan development process using accepted wraparound technologies and techniques
- Documents the plan development process by recording and distributing minutes of CFT meetings, writing progress notes, and updating Individualized Family Plans, Safety Plans, and planning documentation
- Prepares Individualized Family Plans and Safety Plans and coordinates each plan’s implementation, encouraging the maximum use of natural resources and community based services, and supplementing these with flexible wraparound staffing and funding
- Provides and documents all eligible services provided, including but not limited to evaluation, assessment, plan development, collateral services, case management, and individual rehabilitation
- Provides other case management functions as necessary, securing formal resources, and representing the CFTs progress through the wraparound process in written documentation and verbal presentations to both internal and external partners
- Participates in the Internal Utilization Review process and other peer-review mechanisms, providing feedback to colleagues, accepting feedback from colleagues, and promptly responding to recommendations
- Maintains accurate and timely records
- Seeks out clinical information and advice as needed, accepts constructive criticism and reasonable recommendations from supervisors
- Mentors new Specialists
- Becomes certified as a High Fidelity Wraparound Facilitator within 6 months of date of hire
- Ability to work flexible hours to meet the needs of the families
- Participates in the agency 24 hour on-call programs
- Participates in on-going training to expand and develop professional skills
