About this Role:
The Population Health Management Division (PHMD) plays a pivotal role in designing and driving implementation of DHCS' flagship California Advancing and Innovating Medi-Cal (CalAIM) initiative. PHMD is part of DHCS' Quality and Population Health Management Program (QPHM) and, as a two-year old division, operates as a startup within DHCS. PHMD is recruiting an advanced Health Program Specialist II (HPS II), serving as a team member for the Population Health Management (PHM) Policy Section responsible for PHM policy development, implementation, and monitoring.
The HPS II will ensure high-quality program administration; monitor and successfully complete multiple work streams simultaneously; enable DHCS' managed care partners to implement PHM policy with the greatest possible fidelity; and, where appropriate, improve the existing PHM policy for achieving CalAIM goals and outcomes.
This position is eligible for remote hybrid work. Employee may need to report to the Sacramento offices no more than twice a week.
Telework is based on operational needs for employees who can perform their essential functions remotely and/or whose duties are conducive to teleworking per DHCS’ Telework Policy and in accordance with Government Code Sections 14200 – 14203 for employees residing in California.
