Pay Range: $21-24/HR plus shift differential for qualifying shifts

Provides personal care and nursing assistance to residents; assists in daily living activities, provides resident documentation and evaluation.
Responsibilities- Takes and records vital signs: temperature, pulse, blood pressure, and respirations as required; recognizes abnormalities.
- Participates in development of residents’ plan of care and follows current plan of care; promptly reports any change in conditions of residents.
- Demonstrates comprehensive physical assessment skills for initial and follow‑up appraisal of resident status; analyzes initial and ongoing assessments in development of the plan of care.
- Provides and assists residents in bathing/showering for thorough cleanliness and hygiene; assists residents with grooming and hygiene (i.e. shaving, oral hygiene, cleaning, trimming fingernails/toenails) in a safe and thorough manner daily.
- Assists with feeding as necessary; assesses for choking and responds appropriately.
- Assists with toileting in a safe and thorough manner per residents’ plan of care.
- Assists with balancing/standing activities; bed mobility, techniques and ambulation and the use of assistive devices.
- Assists residents in dressing with concern for privacy while providing care.
- Makes beds, straightens rooms, and keeps resident care areas organized.
- Provides safe resident transfers and lifting techniques involving bed, wheelchair, chair and/or shower chair.
- Maintains inventory of medical equipment/devices for residents (wheelchair, assistive devices); ensures equipment is in good condition and notifies maintenance for repair.
- Promotes independence in resident care as appropriate; demonstrates awareness of safety for both residents and the individuals providing care.
- Assists residents with appropriate range of motion or other exercises as directed by nurse or therapist and ambulates residents safely according to his/her endurance levels and care plan.
- Applies medical or adaptive devices as ordered by the appropriate supervising nurse.
