** Physical Therapist (PT)
** to join our dynamic rehab team in a skilled nursing/post-acute care setting. We are looking for a compassionate, motivated clinician who values quality care and teamwork.
** Pay Range:*** **$60–$70/hour**, based on experience
** Schedule Options:
*** Full-time
* Part-time
* PRN* Flexible scheduling available
* Supportive leadership and collaborative interdisciplinary team
* Well-equipped rehab gym
* Stable facility with a strong focus on patient outcomes
* Great Midtown Sacramento location
** Physical Therapist (PT) Responsibilities:
*** Evaluate and treat patients in a skilled nursing/post-acute setting
* Develop and implement individualized treatment plans
* Document evaluations, progress notes, and discharge summaries accurately and timely
* Collaborate with nursing, therapy, and interdisciplinary teams
* Educate patients and families on therapy goals and home exercise programs
** Physical Therapist (PT) Requirements:
*** Active California Physical Therapist license (or pending)
* Skilled nursing or post-acute experience preferred, but not required
* Strong communication and time-management skills
* Team-oriented, compassionate, and patient-focused
