As a full‑time Home Health Speech‑Language Pathologist (SLP) you will provide skilled, compassionate care in patients’ homes. In collaboration with the Case Manager and interdisciplinary care team you will assess, treat, and support patients with speech, language, cognitive‑communication, voice, fluency, and swallowing disorders. You will also act as an educator and patient advocate, promoting improved communication and quality of life.
In addition to your clinical responsibilities you may serve as a Clinical Fellowship Supervisor, mentoring speech‑language pathology clinical fellows toward certification. When supervising a fellow in your area, your weekly visit expectation will be reduced, allowing time for comprehensive training, support and evaluation in line with certification standards.
Key Responsibilities- Provide supervision, guidance, and mentorship to speech‑language pathology clinical fellows during the fellowship year.
- Develop and implement individualized learning plans (ILPs) outlining goals, objectives, and milestones.
- Conduct regular evaluations and assessments of fellows’ progress and clinical competencies.
- Provide constructive feedback and coaching to enhance clinical skills and decision‑making.
- Collaborate with coordinators and other healthcare professionals to facilitate learning opportunities.
- Ensure compliance with certification and licensure requirements.
- Facilitate discussions and case reviews to promote clinical reasoning and evidence‑based practice.
- Coordinate and participate in meetings, seminars, and workshops for professional growth.
- Maintain accurate records and documentation related to fellowship supervision.
- Master’s or Doctoral degree in Speech‑Language Pathology (SLP).
- Current California license or credential as a Speech‑Language Pathologist.
- Certificate of Clinical Competence (CCC‑SLP) from the American Speech‑Language‑Hearing Association.
- Clinical experience as a Speech‑Language Pathologist preferred.
- Experience in supervising or mentoring clinical fellows preferred.
- Strong knowledge of speech therapy principles, practices, and techniques.
- Excellent interpersonal and communication skills.
- Ability to work collaboratively in a multidisciplinary team environment.
- Demonstrated organizational and managerial skills.
