The Care Coordinator, Community Health Worker and Peer Navigator is responsible for developing and providing high quality therapeutic interventions and case management services in support of an individual’s wellness and recovery. Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support.

This is a community-based position providing direct, face-to-face services to individuals who are chronically homeless and travel throughout Sacramento County is required.

At our CORE in Sacramento, every door is the right door when you come to us, so clients who are requesting assistance with housing, employment, behavioral health, food, healthcare, and more are all welcome and will find the support they need at our neighborhood hubs.

Our Crisis Navigation Program (CNP) provides mobile outreach and crisis intervention services to community members of all ages. CNP strives to meet people "where they are at" and provides short-term navigation services for up to 60 days including "warm hand-off" to ongoing mental health services.

The THRIVE programs are new initiatives by Sacramento County Behavioral Health to provide services specifically for adults who are justice involved and meet criteria for specialty mental health services. Justice involved may include participating in mental health diversion, mental health court, parole/probation, etc. THRIVE Outpatient Program will serve the moderate level community members and be open a standard Monday-Friday. THRIVE FSP will serve the high intensity community members and operate seven days a week.

Schedules include:

  • Monday-Friday Day shift for our THRIVE Outpatient Programs.
  • Sunday-Thursday 11:30am-8:00pm for our Crisis Navigation Program.
  • Sunday-Thursday, or, Tuesday-Saturday Day shift for our THRIVE FSP Program.
  • Monday-Friday Day shift for our CORE programs.

Responsibilities

  • Development and delivery of a full complement of services, including individual, family, group counseling, individual rehabilitation and case management/brokerage for individuals within the Care Coordinator's caseload as assigned.
  • Utilize Evidence Based Practice for the designated population/program to assure high quality results and intended impact.
  • Continuously assesses individual’s risk status including risk based on symptoms, substance use, environment, physical conditions, etc and intervening appropriately.
  • Assess the continued service necessity and creating a service plan that aims at an individual getting better/healthy and accomplishing service goals that are person-centered and belong to the client.
  • Continuous outreach and progressive engagement and reporting disengagement to the team immediately.
  • Work with outreach team and determining when to disenroll individual based on program protocols. Responsible for domain-based assessment, screening referrals, service planning, community planning and benefit coordination which supports wellness and recovery principles. Responsible for care planning and therapeutic case management using a whatever it takes approach. Documents and maintains all data collection, reporting, and charting records in accordance with BACS and other relevant county regulatory policies/agencies.

    Responsible for producing required number of regulatory units of service per month (65% of staff’s time must be providing face-to-face or other eligible community-based support). Participates in supervision, staff meetings, consultation, travel, crisis/emergency coverage, weekend coverage, as well as, agency-wide meetings and training, as required by manager. Able to work independently and as a member of a therapeutic team. Is adaptable and open to change and new information;
