Scope

of Work

Capio Group is looking for an experienced Business Solutions Analyst to be part of a team providing Organizational Change Management (OCM) services, consultation, and support for a client Case Management project. The OCM services will help facilitate the successful implementation of the new case management system to support business operations. The project will require engagement with internal and external stakeholders, including client staff, other entities, and external providers depending on needs.

The change management services will include a roadmap for managing change:

  • Baseline Assessment
  • Messaging and Communication
  • Implementation
  • Measuring Success
Responsibilities
  • Change Management Implementation and Delivery:
    Work collaboratively with the client Project Team to implement components of the change initiative.
  • Change Management Evaluation and Closure
  • Documentation:
    All OCM documentation, templates, forms, etc., that are a result of this agreement, and associated deliverables must be flexible and reusable by the client for future projects and initiatives. Documentation must include, but is not limited to, policies and standards. Additionally, agree and adhere to administrative processes supporting action items, issue tracking, risk management, version control, and tracking document changes.
  • Change Management Planning and Assessment:
    Document project goals and objectives; assess the current state of the program, perform a gap analysis of the as-is vs. to-be state, develop a plan to bridge the gap, and develop a methodology and measures to evaluate progress and effectiveness.
  • OCM Project

    Schedule:

    Develop and deliver an overall project schedule addressing all OCM activities, tasks, and deliverables.
  • Change Initiative Gap Analysis:
    Develop and deliver a Gap Analysis with baseline assessment, program readiness, desired state, and recommended actions.
  • Change Initiative Assessment:
    Develop and deliver a methodology and measures to monitor progress and effectiveness.
  • Change Management Plan:
    Develop and deliver a Change Management Action/Communication Plan, including actions, milestones, timelines, and success criteria.
  • Change Management Tasks:
    • Establish branding and core messaging
    • Develop and implement a strategy to roll out the change to stakeholders
    • Address stakeholder questions and concerns
    • Identify and select tools for ongoing communication to client managers, staff, and stakeholders on status and progress
  • Change Management Kick-Off Meetings:
    Facilitate kickoff meetings with the client Project Team and stakeholders to outline purpose, roles, and feedback methods.
  • Change Management Communication:
    Develop and execute a plan to provide regular updates to client managers and staff on status, milestones, and events.
  • Initiative Check-In Meetings:
    Convene regular check-ins with the client Project Manager and managers as needed, at least monthly.
  • Stakeholder Status Updates and Feedback Meetings:
    Convene regular meetings with stakeholders to review project status, milestones, and solicit feedback, as frequently as needed but at least quarterly.
  • Change Management Initiative Success:
    Use defined metrics to measure success and lead corrective actions as needed.
  • Change Management Evaluation Meeting:
    Convene and facilitate meetings to discuss accomplishments, lessons learned, and future change management needs; deliver a Change Management Closure Report.
