This is a 13-week contract position requiring a dedicated nurse with OASIS experience. The successful candidate will be responsible for assessing patient and caregiver needs, developing care plans, managing caseloads, and ensuring compliance with regulatory standards.

• Utilize the nursing process to assess patient and caregiver needs and provide quality care.
• Develop and communicate realistic plans of care in accordance with evidence-based practices.
• Act as a Case Manager, managing caseloads and visit times.
• Complete OASIS documentation timely and accurately.
• Implement medical plans of care and nursing interventions as necessary.
• Stay current with high-tech skills and infection control standards.
• Collaborate with LPNs to ensure the care plan is followed.
• Evaluate the effectiveness of the care plan and make necessary adjustments.
• Educate patients and caregivers about treatments and medications.
• Coordinate care with interdisciplinary teams to address various patient needs.
• Ensure appropriate documentation for agency, state, and federal requirements.
• Participate in case conferences and peer review audits.
• Advocate for patient care and address specific needs of patients and families.

• OASIS experience is required.
• Current nursing license in the relevant state as per Vermont State Board of Nursing regulations.
• Demonstrated knowledge of state and federal regulations related to nursing practice.

The position is scheduled from 8 am to 4 pm, Monday through Friday, with some weekend and on-call requirements.
