AboutThe Role
The Transportation Support Specialist (TSS) is responsible for providing transportation to individuals for scheduled appointments in an effort to support individuals’ overarching treatment goals and engagement in medically necessary services. This may include supporting someone to arrive at their employment site and home from the employment site, safely. The TSS may also provide minimal amounts of pre and post shift supports as needed to some individuals.
PrincipalResponsibilities
- Provide transportation to individuals so they can engage in medically necessary services, maintain employment, and maintain community independence.
- Interacts with individuals served with respect and in a professional and dignified manner.
- Respects individuals’ choices and encourages advocacy.
- Demonstrates knowledge of individual characteristics such as physical health, mental health, psychosocial characteristics in order to address needs in a personalized manner.
- Ensures the safety and wellbeing of individuals served.
- Interacts with team members, community and support services appropriately to ensure continuity of services.
- Communicates appropriate information, issues and concerns in a timely manner to their direct supervisor.
- Maintains confidentiality according to agency policy and federal and state law.
- Reports abuse and or neglect as a mandated reporter to the supervisor and other appropriate reporting entities.
- Maintains flexibility and is responsive to changes in daily routine.
- Accommodates physical needs of individuals.
- Documents required paperwork (i.e. daily data, service logs, timesheets, mileage sheet…etc.) in a timely manner.
- Participates in meetings, trainings, and supervision as scheduled.
- Other duties as assigned.
- High School Diploma or equivalent (GED).
- Previous experience in Mental Health, Substance Use Preferred.
- Proficiency with computers, experience entering data into electronic medical records (EMR) program may be required.
- Ability to respond to emergency situations appropriately.
- Ability to work with others in a positive, responsible manner.
- Strong writing and organizational skills in order to receive and convey information in an efficient and effective manner.
- Ability to maintain confidentiality.
- Flexibility, dependability, and ability to work independently.
- Commitment to community inclusion and individual choice.
- Able to flex schedule as needed.
- Must have a valid driver’s license with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
- Reliable transportation highly preferred.
- Successful completion of a criminal history background check.
- Competitive Pay -$20.00 / hour
