Transitional Care Management Nurse coordinating care for patients transitioning from inpatient care or facility‑based care to the outpatient setting. This role focuses on reducing hospital readmissions ensuring continuity of care and supporting patients during high‑risk post‑discharge periods in compliance with CMS TCM requirements. Works as an advocate for the physical and emotional well‑being of the patient.

- As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.
- Education:
Graduate of a Practical Nursing program. - Licenses:
Current LPN License to practice in the State of Arkansas. - Certifications:
AHA Basic Life Support (BLS), within 30 days of hire.
- Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Must be able to work in a stressful environment and take appropriate action.
