Registered Nurse (RN) - Case Manager (Days, Full-Time)
Location: Russellville Hospital - Russellville, AL
The Staff Pad has partnered with Russellville Hospital in Russellville, AL to hire a dedicated RN Case Manager
. This role is responsible for coordinating and managing patient care throughout the hospital stay, ensuring quality, efficiency, and safe transitions to home or post-acute care. The RN Case Manager works closely with physicians, nursing staff, patients, and families to optimize outcomes and facilitate smooth, patient-centered care.
Key Responsibilities:
- Assess patients and develop individualized care plans in collaboration with the healthcare team
- Coordinate patient care across interdisciplinary teams to ensure continuity and quality
- Facilitate discharge planning, including referrals to home health, rehab, and community resources
- Monitor and track patient outcomes and identify opportunities for improvement
- Educate patients and families regarding diagnoses, treatments, and follow-up care
- Ensure compliance with hospital policies, regulatory standards, and documentation requirements
- Act as a patient advocate, addressing questions and concerns throughout hospitalization
- Current Alabama RN license
- Bachelor of Science in Nursing (BSN) preferred
- Minimum 3 years of hospital experience in Med-Surg, Case Management, or related setting
- Strong communication, critical thinking, and organizational skills
- Knowledge of care coordination, utilization review, and discharge planning processes
- Full-time, Days
- Standard hospital hours (Monday-Friday, may include occasional weekends if needed)
