Job Summary and Qualifications

The Clinical Excellence Coordinator coordinates clinical excellence initiatives (EX. Sepsis, Mortalities/Complications, CHIRP, etc.) along with the Medical and Clinical staff. The coordinator uses a multidisciplinary approach and acts as a liaison between physicians, nursing, and staff. The coordinator identifies learning needs and the content for education of the clinical excellence initiatives. The coordinator develops, presents, coordinates, and evaluates education activities. The coordinator effectively performs in the role of facilitator, advocate, consultant and/or researcher.

General

Responsibilities
  • Lead the coordination, dissemination, and implementation of clinical excellence initiatives to achieve compliance and improve quality outcomes.
  • Maintain extensive knowledge of clinical excellence initiatives and related data definitions
  • Perform concurrent review of patient care specific to initiative as needed
  • Collaborate and educate physicians, nurses, and ancillary staff to facilitate care management to achieve compliance with clinical excellence initiatives and improve quality outcomes
  • Read, analyze, and summarize medical information for appropriateness of medical management related to clinical excellence initiatives.
  • Monitor performance through use of data sources such as facility, division, and corporate reports
  • Monitor initiative-specific dashboards/reports and validate accuracy of data
  • Identify opportunities for improvement
  • Analyze medical records for potential performance issues
  • Collaborate with key players to form actions for improvement and make necessary changes
  • Educate staff (medical, clinical, and ancillary) on changes to clinical initiatives and need for process improvements based on performance.
  • Identify need for education/re-education in areas specifically related to patient care.
  • Identify key players for education/re-education.
  • Assist in development and presentation of education/re-education to appropriate staff
  • Monitor efficacy of education/re-education to drive improvement of outcomes
  • Assist Quality Director with special projects as needed.
Knowledge,

Skills and Abilities
  • Sets priorities, makes complex decisions, and conducts on-going evaluation of initiative progress and takes necessary action.
  • Interfaces directly with physicians, nursing leadership, nursing staff, other healthcare team members, and executive leadership members throughout St. David’s Round Rock Medical Center.
  • Excellent communication and presentation skills (written and verbal).
  • Must be a self-starter and self- motivated with the ability to work independently under minimal supervision.
  • Proficient knowledge of word-processing, spreadsheets, and data entry.
  • Capability of interpreting data and creating understandable information.
Education and Experience
  • Bachelor’s degree in nursing required; MSN, MHA, or MBA preferred
  • 2 years of RN experience
Licenses and Certifications
  • Registered Nurse license in Texas. If Compact License, Texas license must be obtained within 90 days of hire
