Psych Nurse Practitioner - Round Rock, TX – Part-Time (20651)

Join Cantex as a Psych Nurse Practitioner with a focus on mental health and chronic disease management. This part‑time position allows you to provide patient care within an interdisciplinary team while maintaining high standards of practice and confidentiality.

Job Summary

The Nurse Practitioner collaborates with physicians, RNs, social workers, and case managers to deliver holistic, compassionate care to members and families. The role includes diagnosing, managing, and preventing acute and chronic illnesses, and developing comprehensive care plans in partnership with a supervising physician.

Essential Functions
  • Performs in‑depth health risk assessments for all assigned members.
  • Collaborates with physicians, case managers, social workers, family members, and other ancillary personnel.
  • Identifies quality improvement opportunities and plans individualized member care according to policies and protocols.
  • Reviews medical, surgical, and social histories and interprets diagnostic data.
  • Performs appropriate screening procedures and recommends treatments, including prescribing medications, injections, wellness counseling, suturing, and ancillary testing.
  • Refers members to PCP, case management, and disease management programs as needed.
  • Maintains PHI confidentiality in compliance with HIPAA.
  • Diagnoses conditions and creates comprehensive care plans.
  • Achieves clinical performance targets.
  • Creates positive rapport and engagement with members.
  • Communicates and collaborates with facility leadership to meet member care goals.
  • Participates in clinical committees for guidelines, quality improvement, and compliance.
  • Schedules team member availability to ensure continuity of care.
Qualifications
  • Minimum 2 years clinical experience as a Nurse Practitioner in geriatric, adult, or family practice.
  • Ability to perform comprehensive, adult/geriatric physical exams.
  • Experience diagnosing and documenting chronic disease in adults.
  • Recognition of risk factors relevant to seniors.
  • Commitment to continuing education and professional development.
  • Medication reconciliation proficiency.
  • Documentation of complex member needs.
  • Relevant clinical work experience for the served population.
Education
  • Graduated from an accredited Adult/Gerontological Primary Care NP program.
Licensure/Certification s
  • Current RN and NP licensure in the state of practice.
  • Board certification byANP or ANCC in Adult, Family Practice, Geriatrics or Adult‑Gerontology Primary Care.
  • Current CPR certification.
Preferences
  • Geriatric certification (or to be obtained within 18 months of employment).
  • Bilingual in English and Spanish.
  • Knowledge of community resources.
  • Experience working with behavioral health populations.
  • Experience with low‑income and disadvantaged populations.
  • Experience with vulnerable adults or senior population.
Travel Requirements
  • Must meet company driving standards and have a valid state driver’s license.
  • Must be able to independently travel to assigned locations.
  • Must provide proof of automobile liability insurance coverage.
Physical and Psychological Demands
  • Prolonged sitting, bending, stooping, twisting, kneeling, crouching, crawling, and stretching.
  • Manual dexterity for operating medical equipment, computer, and office equipment.
  • Eye‑hand coordination and the ability to adjust visual focus.
  • Effective use of telephone or computer for communication.
  • Moderate to intense concentration due to job complexity.
  • Ability to lift or move up to 50 lbs.
  • Adherence to Universal Precautions for blood‑borne pathogens.
Work Environment

The work environment includes intermittent exposure to variable temperatures, fumes, airborne particles, blood, and other body fluids. Noise is usually moderate, and the schedule alternates between administrative and clinical duties based on facility needs.
